Outcome Measure QuestionnairesInjury Questionnaires
Standard patient-reported outcome measures for workers compensation, CTP, DVA and private clients. Complete a questionnaire, score it, compare against a previous result and download a PDF report. Offline and self-contained — nothing is sent anywhere.
Please complete these questionnaires before your appointment. They help your physiotherapist understand your injury and track your recovery. Your answers stay on your device — nothing is sent anywhere until you email the PDF report to us.

Evidence & selection
Measurement properties, change thresholds and licensing for each questionnaire. MCID = minimal clinically important difference; MDC = minimal detectable change (the smallest change that exceeds measurement error).
How questionnaires are selected
The triage on the Client details page applies these rules. The clinician can tick or untick any questionnaire; manual changes are marked in amber.
| Trigger | Questionnaire | Rationale |
|---|---|---|
| Every client | NPRS, PSFS | Pain intensity and client-nominated function apply to any region |
| Neck · Headaches · Low back | NDI · HDI · ODI | Region-specific disability |
| Shoulder · Elbow/forearm · Wrist/hand | SPADI · QuickDASH · PRWE | Region-specific; QuickDASH added when two or more upper-limb regions are selected |
| Hip · Knee · Ankle/foot | HOOS · KOOS · FADI | Region-specific; LEFS added when two or more lower-limb regions are selected |
| Upper/mid back or Other only | NPRS, PSFS | No validated region-specific PROM; PSFS is the primary function measure |
| First assessment + workers compensation, CTP or off work | Örebro (short) | Screens risk of long-term disability and work absence (Linton 2011) |
| First assessment + low back, not compensable and not off work | STarT Back | Low back risk stratification (Hill 2011); Örebro replaces it when work disability is the question |
| First assessment + compensable, off work or symptoms ≥12 weeks | K10 | Distress is a recognised obstacle to recovery in these groups |
| Symptoms ≥12 weeks, or off work ≥4 weeks | PSEQ, PCS | Self-efficacy and catastrophising as treatment targets in persistent pain |
| Review visit | Region measures, NPRS, PSFS, plus any questionnaire in the loaded history | Repeats outcome measures for comparison. STarT Back and Örebro are screening tools and are not repeated unless ticked |
| After scoring | Örebro ≥50 → K10, PSEQ, PCS · STarT high risk → PSEQ, PCS · NDI headache section ≥3 → HDI | Offered as one-click additions on the questionnaire page |
DASS-21 is never auto-selected, to avoid duplicating the K10. Add it manually when subscale information (depression, anxiety, stress) is needed. The 12-week and 4-week thresholds are pragmatic markers of persistence, not validated cut-points.
Suggested selection by presentation
| Presentation | Primary measure | Add |
|---|---|---|
| Neck pain / whiplash | NDI + NPRS | PSFS; HDI if headache is a feature; Örebro at intake for compensable claims |
| Low back pain | ODI + NPRS | STarT Back at first visit for stratification; Örebro for work-related claims; PSFS |
| Shoulder | SPADI | QuickDASH where the whole upper limb is involved; NPRS |
| Wrist / hand / forearm fracture | PRWE | QuickDASH; NPRS |
| Elbow / multi-region upper limb | QuickDASH | NPRS; PSFS |
| Hip | HOOS | LEFS; NPRS |
| Knee | KOOS | LEFS; NPRS |
| Foot / ankle | FADI (+ FADI Sport for active clients) | LEFS; NPRS |
| Psychosocial screening | Örebro short form at intake | K10 or DASS-21 for distress; PSEQ for self-efficacy; PCS where catastrophising is suspected |
Cross-cutting caveats
- MCIDs are context-dependent. Published thresholds vary with condition, baseline severity, acuity and the anchor used. The value applied by this tool is stated beside every comparison. Treat it as a guide, not a pass mark.
- MDC before MCID. Where a published MDC exceeds the MCID, a change can reach the MCID and still sit within measurement error. The tool reports both when both are published.
- Screening tools are not outcome measures. STarT Back and the Örebro short form stratify prognostic risk. Re-administering them shows whether the risk category has shifted, not how much function has improved.
- Severity bands are not universal. Only NDI, ODI, HDI, K10, DASS-21 and PCS have published severity categories. For KOOS, HOOS, FADI, LEFS, QuickDASH, SPADI and PRWE the tool reports the score and change only — it does not invent bands.
- Compensable context. Scores in compensable populations tend to be higher at baseline and to change more slowly. A high score is not evidence of symptom amplification on its own; interpret it alongside examination findings.
- Electronic administration and licensing. Most instruments here are free for clinical use. ODI, QuickDASH, PCS and PSEQ are copyrighted, and some licensors require permission for electronic or web-based administration. Confirm the terms before hosting this tool on a public website.
- Wording. KOOS, HOOS and SPADI item wording follows the standard published English versions. Imperial units in ODI, PRWE, SPADI and LEFS are retained with a metric equivalent in brackets.
How to use
The PDF report is the record. It carries the full results and a results code that this tool reads back at the next visit.
Client completing at homeCompleting your questionnaires
- If the clinic sent you a personal link, your details are already filled in. Check them, answer question 4 about your work, click Start questionnaires and continue from step 4.
- Enter your details. On 1 · Client details1 · Your details, type your name, date of birth and today's date.

- Answer the questionnaires. Answer every question, then click Next questionnaire. When the last one is done, the report page opens.
- Save the report as a PDF. Open 3 · Summary & compare3 · Your report and click Download full report (PDF). In the print window, set Destination to Save as PDF and click Save.
- Email the PDF to the clinic. Attach the saved PDF to an email to your physiotherapist.Email the saved PDF to admin@alliedhealth.net.au, attaching it to the email.
Clinician — at each visit
- Save the client's PDF. Save the emailed PDF to the client's file. This is the record you use for insurer forms.
- Copy the results code. Open the client's most recent PDF, go to the last page and select all of the text in the Results code box. Copy it.
- Paste it into the tool. Open this tool, go to 1 · Client details, paste the code into Results code from previous PDF and click Load results code. The client's name and date of birth fill in, and a green message lists the questionnaires and dates loaded.
- Check the triage. The injury sites and injury date fill in from the code, and the assessment is set to review. Update the work status, then check the ticked questionnaires under Questionnaires to complete and tick or untick any you want to change.
- Complete today's questionnaires. Click Start questionnaires and have the client answer each one, clicking Next questionnaire after each. Each previous score appears automatically in the comparison box under the score. If a Recommended to add box appears, click Add.
- Save today's PDF. Open 3 · Summary & compare, click Download full report (PDF), set Destination to Save as PDF and click Save. Its results code contains every visit to date, so next time you only need this newest PDF.
